Medical Guidelines for Sports Participation 2008-2009
School Year
Every student is required to have an annual physical
prior to tryouts.
This physical must be documented on the
Sports Participation form.
This
form is available from
the nurse's office or on-line through the SHA athletic
web page.
The physical
needs to be completed after
June 15, 2008 and returned to Sacred Heart Academy
before August 15, 2008.
Please be certain the form is completed in its entirety
and signed by the examining practitioner, parent, and
student. The school nurse will review and clear students
for tryouts in August. Since physicals will be completed
during the summer months, the student will be cleared
for the entire school year. Each physical is valid for
one calendar year only.
We recommend retaining a copy for your personal records
prior to mailing the form
